The Risks of Therapy Chatbots: Insights from Stanford University’s Research

The rise of therapy chatbots powered by large language models (LLMs) has sparked both excitement and concern within the mental health community. While these digital companions have been hailed for their potential to make mental health support more accessible, recent research from Stanford University unveils significant risks that accompany their use. The study, titled “Expressing stigma and inappropriate responses prevents LLMs from safely replacing mental health providers,” delves deep into the implications of relying on AI for therapeutic support and raises important questions about the future of mental health care.

The Stigmatization of Mental Health Conditions

In a landscape where mental health remains heavily stigmatized, the findings from Stanford’s research are particularly alarming. The study assessed five chatbots designed to provide therapeutic support and explored how these AI systems respond to users with various mental health conditions. During their experiments, researchers presented vignettes containing symptoms associated with different conditions and gauged the chatbots’ responses.

The results were troubling, revealing that these chatbots displayed increased stigma toward certain conditions—such as alcohol dependence and schizophrenia—compared to others like depression. Jared Moore, the paper’s lead author and computer science Ph.D. candidate, emphasized that “bigger models and newer models show as much stigma as older models.” This suggests that advancements in AI technology do not automatically translate to more compassionate or understanding responses.

Inappropriate Responses to Critical Situations

Equally concerning is the finding that these chatbots sometimes failed to respond appropriately in high-risk scenarios. In the second experiment conducted by the researchers, real therapy transcripts were used to evaluate the chatbots’ handling of sensitive topics like suicidal ideation and delusions. Shockingly, when a user mentioned losing their job and asked about tall structures in New York City, chatbots like 7cups’ Noni and Character.ai’s therapist merely provided information without addressing the underlying emotional distress. This failure to engage meaningfully could lead to dangerous outcomes for individuals in crisis who might rely on these chatbots for support.

The Role of AI in Therapy: A Cautionary Perspective

Although the study illuminates significant shortcomings in using chatbots as substitutes for human therapists, it also opens the door for reconsideration of their role in mental health care. As Nick Haber, an assistant professor at Stanford’s Graduate School of Education and a senior author of the study, noted, LLMs could potentially fulfill supportive roles—such as assisting with billing, providing training materials, or helping patients with tasks like journaling—rather than acting as stand-ins for qualified professionals.

“LLMs potentially have a really powerful future in therapy, but we need to think critically about precisely what this role should be,” Haber articulated. This perspective encourages mental health practitioners and technologists to collaborate in defining effective and safe applications for AI in therapeutic settings, rather than allowing chatbots to operate in a vacuum.
