Leanna Fortunato, a licensed clinical psychologist and director of quality and health care innovation for the American Psychological Association, points out that discussions around AI in therapy are becoming more prevalent. “Anecdotally, providers are talking about it, and we know from the research that people are using AI tools for support more and more,” she states.

A recent health research survey involving over 20,000 U.S. adults revealed that 10.3% of them engage with generative AI daily. Of this group, a staggering 87.1% use the technology for personal reasons, including advice and emotional support. However, these AI companions can often lead users into mental health conversations without clear guidance.

The Popularity and Risks of AI Chatbots

On social media platforms like TikTok, hashtags related to "Therapy AI Bot" boast over 11.5 million posts. Users share prompts to optimize their interactions with chatbots, but experts warn about potential dangers. AI chatbots have historically struggled to recognize moments of genuine distress. A report from The New York Times highlighted nearly 50 cases where users experienced mental health crises during conversations with ChatGPT, including three tragic fatalities.

Companies like OpenAI and Google are aware of these grave issues and are actively collaborating with mental health professionals to enhance their chatbots’ responses in sensitive situations. “We continue to improve ChatGPT’s training to recognize signs of distress and guide people toward real-world support,” an OpenAI spokesperson noted.

The Impact of AI on Social Skills and Loneliness

Frequent reliance on AI companions can have adverse effects on real-life social skills. Studies published by OpenAI and MIT Media Lab suggest that heavy use of AI chatbots correlates with increased feelings of loneliness. The American Psychological Association warns against viewing AI as a substitute for professional therapy or mental health support.

Using AI Responsibly: A Tool, Not a Therapist

Mental health professionals like Esin Pinarli view AI chatbots as potential tools rather than replacements for therapy. “I see it as a tool, and I think that a tool can be helpful,” she says. Pinarli suggests using chatbots for generating journaling prompts, learning about mental health topics, and asking for research links—but not for personal advice or diagnosis.

Advising caution, Fortunato encourages users to cross-check AI-provided information with reputable sources, recognizing that AI can enhance access to mental health resources but doesn’t guarantee accurate guidance.

Key Considerations When Interacting with AI Chatbots

  1. Crisis Support: Never rely on AI for support during a mental health crisis. Contact a professional or a crisis line like the Suicide and Crisis Lifeline (988), available 24/7.

  2. Confidentiality Matters: Avoid sharing personal or medical information with chatbots; these conversations lack legal confidentiality.

  3. The Human Element: Emotional needs often require human interaction. AI lacks the ability to interpret body language and tone, crucial elements in meaningful conversations.

  4. Research Before Action: Validate any advice or information received from AI by consulting a licensed professional or reliable health sources.
